New Patient Registration

You can register online to become a new patient at Carnon Downs Surgery if you live within our catchment area.

If you are a new patient and would like a copy of our practice leaflet please ask at reception but our website has all the current and most up to date information.

Register with us

Complete the online form to register here at the surgery.

If you complete the forms at the desk you will also need to provide a copy of your ID and proof of address. For example, valid ID could include a photograph of your passport or driving licence. This is to check that you live in the local catchment area.

We may also ask you for details of your previous GP surgery and basic health and medical information.

  • Please note Children under 16 need to be registered by their parent or guardian. You can do this by completing the online GP surgery registration form on their behalf.
If you are a carer of young children, elderly adults or adults who aren’t able to register themselves, you can set up an account on their behalf. This will allow you to book an appointment and access parts of their GP online services account.

Catchment Area

As part of our serivce to you, we would like to inform you that you have a named allocated GP which is recorded on your medical record. If you would like to know who your named allocated GP is, please ask Reception.

This does not mean that you cannot see a GP of your choice as we do not hold individual GP lists and you are registered with the Practice in general.

Registering as a temporary patient

If you are away from home, you can register as a temporary patient with Carnon Downs Surgery for up to three months.

This will allow you to use our surgery and still remain a patient at your permanent GP surgery. After three months, you will have to re-register as a temporary patient or permanently register with us.

You can register as a temporary patient at Carnon Downs Surgery by calling reception.

Change of Patient Details

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you will be asked to register with a surgery that’s closer to your new home address.

You can fill in our form to let us know about a change in your address or contact details.

Registering online as a non-English speaker

You can find guidance and advice on how to become a new patient and the NHS healthcare services you can access, if your first language is not English, or if you’re an asylum seeker on the Government website.
